Plan Description:
- Comprehensive insurance plan with indexation benefits.
- Sum Assured / Coverage: Minimum BDT 5 Lakhs to Maximum BDT 20 Lakhs.
- Policy Term: 5 to 20 years.
- Payment Mode: Yearly.
- Entry Age Limit: 18 to 55 years.
- Exit Age: 65 Years.
- Premium: Depends on age, policy term and coverage value.
- 10% increase in sum assured for every claim-free year (indexation benefits).
- There will not be any medical test required for the issuance of this policy.
- The product will be offered on accept/reject basis.
Base Benefit: On diagnosis of –
- Major-Stage Cancer – 100% of the Sum Assured payable as lump sum benefit less any Early or CIS benefit already paid.
- Early-Stage Cancer or Carcinoma-in-situ (CIS) – 25% of the Sum Assured payable as a lump sum benefit.
Indexation Benefit:
- Under the Indexation benefit, the Sum Assured increases by 10% of the initial Sum Assured for every claim-free year starting from the first policy anniversary.
- The maximum increase allowed will be equal to 100% of the original Sum Assured. The indexation benefit will cease in the event of a valid claim.
Once the claim is made, all future claims shall be based on the Increased Sum Assured at the time of the first claim and further increases to the Increased Sum Assured shall not be applicable.
Waiver of Premium Benefit:
- On a valid claim of Early-Stage Cancer or Carcinoma-in-situ (CIS), premiums will be waived for a period of 3 policy years. In case the outstanding policy term is less than 3 years, premiums for the outstanding policy term would be waived.
- Following the 3-year period, the policyholder has to resume paying the premiums for the outstanding policy term (if any).
Waiting Period:
- A waiting period of 180 days is applicable from the date of commencement or the date of reinstatement of cover.
- No benefit will be payable during the waiting period. In the event of a valid claim, the premiums paid will be forfeited.
- If the policy is reinstated within 30 days of lapse, the 180 days waiting period will not be applicable, provided the policy has been in-force for at least 180 days.
Survival Period:
- A survival period of 7 days is applicable post-diagnosis before any claim can be filed (early stage or major stage of Cancer).
Policy Exclusions: No benefit shall be payable under the policy with respect to any Major-Stage Cancer, Carcinoma-in-situ or Early-Stage Cancer resulting directly or indirectly from or caused or contributed by (in whole or in part):
- Any congenital conditions
- Any pre-existing disease (refer to definition below)
- Intoxication by alcohol or narcotics or voluntarily taking or using any drug, medication or sedative unless it is an "over the counter" drug, medication or sedative has been taken according to package directions.
- Nuclear, biological or chemical contamination (NBC)
The policy shall terminate and premiums paid shall be forfeited if the diagnosis or signs and symptoms (related to the diagnosed cancer) first occurred during the waiting period.
Early-Stage Cancer: Early-Stage Cancer shall mean the presence of one of the following malignant conditions:
- Tumour of the thyroid except Papillary Thyroid Carcinoma histologically classified as T1N0M0 according to the TNM classification
- Prostate Cancer that is histologically described using the TNM Classification as T1a or T1b or Prostate cancers described using another equivalent classification
- Chronic lymphocytic leukaemia classified as RAI Stage I or II
- Basal cell and squamous skin cancer that has spread to distant organs beyond the skin
- Hodgkin's lymphoma Stage I by the Cotswolds classification staging system
*The diagnosis must be based on histopathological features and confirmed by a Pathologist. Pre-malignant lesions and conditions, unless listed above, are excluded.
Carcinoma-in-situ: Carcinoma-in-situ shall mean a histologically proven, localized pre-invasion lesion where cancer cells have not yet penetrated the basement membrane or invaded (in the sense of infiltrating and/or actively destroying) the surrounding tissues or stroma in any one of the following covered organ groups, and subject to any classification stated:
- Breast, where the tumour is classified as Tis according to the TNM Staging method
- Corpus Uteri, Vagina, Vulva or Fallopian Tubes where the tumour is classified as Tis according to the TNM Staging method or FIGO* Stage 0
- Cervix Uteri, as Tis according to the TNM Staging method or FIGO* Stage 0
- Ovary –include ovarian tumours with intact capsule, no tumour on the ovarian surface, classified as T1aN0M0, T1bN0M0 (TMN Staging) or FIGO 1A, FIGO 1B
- Colon and Rectum
- Penis
- Testis
- Lung
- Liver
- Stomach and oesophagus
- Urinary Tract, for the purpose of in-situ cancers of the bladder, stage Ta of papillary carcinoma is included
- Nasopharynx
For purposes of this Policy, Carcinoma-in-situ must be confirmed by a biopsy.
* FIGO refers to the staging method of the Federation Internationale de Gynaecologic et d'Obstetrique Pre-malignant lesions and Carcinoma-in-situ of any organ unless listed above are excluded.
Major-Stage Cancer: A malignant tumour is characterized by the uncontrolled growth & spread of malignant cells with invasion & destruction of normal tissues. This diagnosis must be supported by histological evidence of malignancy & confirmed by a pathologist. The term cancer includes leukaemia, lymphoma and sarcoma.
The following are excluded -
- Tumours showing the malignant changes of carcinoma in situ & tumours which are histologically described as premalignant or non-invasive, including but not limited to Carcinoma in situ of breasts, Cervical dysplasia CIN-1, CIN - 2 & CIN-3.
- Any skin cancer other than invasive malignant melanoma
- All tumours of the prostate unless histologically as having a Gleason score greater than 6 or having progressed to at least clinical TNM
- classification T2N0M0.
- Papillary micro-carcinoma of the thyroid less than 1 cm in diameter
- Chronic lymphocytic leukaemia less than RAI stage 3
*All tumours of the urinary bladder are histologically classified as T1N0M0 (TNM classification) or below.
Pre-Existing Disease: Pre-existing Disease means any condition, ailment, injury or disease:
- That is/are diagnosed by a physician within 5 years (*Which is 2 years for BDT 5 Lakh Sum Assured) prior to the effective date of the policy issued by the insurer or its reinstatement or
- For which medical advice or treatment was recommended by, or received from a physician within 5 years (*Which is 2 years for BDT 5 Lakh Sum Assured) prior to the effective date of the policy issued by the insurer or its reinstatement.
Death Benefit:
- No death benefit is payable under the policy.
Maturity Benefit:
- No maturity benefits & paid-up value is payable under the policy.
Surrender Benefit:
- No surrender benefit is payable under the policy.
Settlement of Claim:
- The company will settle the claim as per conditions on the basis of diagnosis of Cancer/Carcinomas-in-situ and subject to submission of necessary medical reports & physician’s opinion.
Claim Application Process- Online Method: Browse the claim portal from the EasyLife/Guardian Life website for online claim submission, and then select the claim button with the following information:
- Insert Policy ID
- Insert a 6-digit OTP to verify your mobile number (which was given at the time of the policy's inception).
- Select the claim type.
- Insert the dates of hospitalization and discharge.
- Input the claim amount.
- Upload the required documents.
- Copies of the doctor’s prescriptions
- Copies of diagnostic reports
- Copy of all bills and money receipts
- Discharge certificate
Claim Application Process-Offline Method:
- Call Easylife’s hotline number: 16622, to notify them of the claim by the policyholder.
- An executive will call back that number and verify some information, in line with the online claim request.
- The Guardian Life Insurance Executive shall then guide the insured to send the necessary documents.
- It can be a soft copy by email/portal upload or a hard copy of any documents (if required) by post or courier, depending on the claim type.
After examining the documents, the insured shall be informed if it needs any clarifications or supporting documents. All submitted documents shall be verified by contacting the insured and/or other concerned persons or organizations if required. If there is no discrepancy in the submitted documents, a claim shall be settled, disbursed, and communicated accordingly with the insured.
Declaration
I hereby declare that I am in *Good Health. I also hereby agree to the Terms & Conditions (As per the Policy Document) of this policy and confirm that all the information and documents provided are true and correct. I also hereby authorize Guardian Life Insurance Limited and its representatives to contact me and to share my information on a confidential basis with third parties for evaluating & processing this proposal and future investigation(s) that may arise.
Good Health: I am currently not receiving any treatment, have not been treated or have not been told to have any treatment for Cancer/AIDS/Kidney, Liver or Lung Disorder/Brain Diseases/Heart or Blood Diseases and I am not currently totally or partially disabled to work due to sickness or an accident and I do not have any physical impairment or psychiatric illness or any mental or nervous disorder. I also declare that I have not been (1) totally disabled to work due to a sickness or accident for 15 days or more continuously during the past five years(*2 years for BDT 5 Lakh Sum Assured); (2) hospitalized for 5 or more days continuously during the past five years (*2 years for BDT 5 Lakh Sum Assured); (3) treated for alcohol or drug abuse.
I hereby also declare that I have not furnished any false information or concealed any material fact in my application. If any concealment of material fact or misstatement is proved in future or at the time of settlement of claims, this contract shall be cancelled from the beginning of this policy, deposited premiums will be forfeited and the claim shall be repudiated.
